Here is a short list of reminders/expectations to allow for an effective first visit:

• Please arrive 10-15 minutes early to allow for adequate time to fill out any unfinished paperwork

• Please bring your medical records with you (or have them faxed to us). We also ask that you bring a list of any medications or supplements you are currently taking. These items will help us better understand your health history and ensure we are providing you with the best possible care.

• We will automatically bill your insurance but if you want to pay out-of-pocket, please call our office to discuss cash pricing.
